Hanna's Health Initiative
The primary goal of the Community Health Initiative is to identify community member’s needs in social services. In addition, bridging the gap between providers of health, social and community services and the undeserved and hard-to-reach populations within the community.  Provide outreach, education, referral and follow-up, case management, advocacy and home visiting services to those who are at highest risk for poor health outcomes. A 2007 Health Needs Assessment Study of the Minority Population in Tippecanoe County showed a need for prevention. Thanks to our partnership with the Minority Hearlth Caoition these needed programs are possible.  

Program Overview

The Hanna Health Initiative seeks to improve the health and well-being of its ever growing population of racial and ethnic minority    populations by implementing programs, services and activities that aim to educate and make communities aware of targeted health priorities and steps that can be taken to improve their over all health. Currently minorities continue to suffer more from preventable diseases such as heart disease, stroke, cancer and diabetes than non-minorities. It is Hanna’s goal to implement programs and services to address these topics to help reduce  barriers, while improving health outcomes.

Our objective is to target Racial and Ethnic Minorities in  the Greater Lafayette Area and its surrounding communities. Hanna’s Health Initiative along with Indiana Minority Health Coalition are committed to  supporting programs and    services that will reduce racial and ethnic health disparities in Tippecanoe County and improve health outcomes within those groups. Target population  are African American, Hispanic/Latino and our two most vulnerable groups, Seniors and children.

Prevention Programs

Adult:

Children:

Operation Fit Kids, Shining Smiles, A lifetime of Healthy Smiles, Teeth to Treasure and Watch Your Mouth

Education and Awareness Activities:

Health  Fairs, Chronic Disease, Immunizations, Mental Illness, Obesity, Sexually Transmitted Diseases, Environmental Health, Vision Health, Oral Health, Tobacco Prevention and Cessation, Health Screens.

Interpretation/Enrollment:

Interpreter is available to families for health appointments and enrollment services for health insurance. Enrollment services  are  available for those individuals needing assistance with health related enrollment forms. Services are available at no cost to the individual.


Referral:


Referral services and follow-up is also available to ensure that individuals have adequate access to needed services in the community
